Review QR codes — collect customer reviews with a scan

A review QR code opens a hosted review form: the customer picks a rating on a 5- or 10-point scale, adds a comment, and optionally leaves an email address. QRCode Suite hosts that page, so there is nothing to build and nothing to connect — no store, no plugin, no CRM. Every response lands in your Leads inbox and exports to CSV. Review Collection codes are part of the Business plan.

Print it on packaging, receipts, table tents or insert cards. The form is served from the code's own link, so the same printed code can be re-pointed or reworded later.

A 5- or 10-point rating scale

Choose the scale when you create the code. The form renders one chip per point, so a customer rates in a single tap before typing anything. The rating is required — the rest of the form is not.

A comment box, not just a score

Below the rating sits an optional free-text field of up to 1,000 characters. The most useful feedback usually arrives here, and it reaches you whether or not the customer leaves any contact details.

Optional email, explicit consent

The email field is optional on review forms, and a separate consent checkbox states plainly that the details are stored so you can get back in touch. Nothing is collected silently or by default.

Works on packaging, receipts and tables

One printed code works wherever the customer already is: the box the order arrived in, the foot of a receipt, a table tent, a thank-you card in the parcel. No app and no login on their side.

Name the product being reviewed

Set a product name and a custom prompt per code, so someone scanning the code on one product sees that product named at the top of the form instead of a generic heading.

First-party reviews that you hold

Responses land in your own Leads inbox and export to CSV. QRCode Suite does not post to Google, Trustpilot or any review site for you — what you get is feedback you hold directly.

    Create a Review Collection code

    Add a QR code in the dashboard and pick the Review Collection type. Set the product name, the prompt shown above the form, and whether the rating runs 1–5 or 1–10.

    Download it and send it to print

    Export a print-ready PNG or SVG and hand it to your printer along with the packaging, receipt template, insert card or table-tent artwork.

    The scan opens the hosted form

    No app, no download, no account for the customer. The page is served by QRCode Suite at the code's own address and works with any phone camera.

    Responses collect in the Leads inbox

    Each submission is stored with its rating, comment, optional email and consent flag — timestamped and attributed to the code that produced it.

    Export the feedback or act on it

    Filter the inbox by code and export to CSV for a spreadsheet, a support queue, or whatever you already use. The printed code keeps working while you reword the prompt behind it.

Frequently asked questions

Do I need to connect a store to use these QR codes?

No. Review, wholesale inquiry and loyalty codes serve a page hosted by QRCode Suite at the code’s own address. There is no store to connect, no plugin to install and no CRM to wire up — which is what lets you print one on a carton or a receipt for a channel you do not control.

Where do the responses go?

Into the Leads inbox in your dashboard. Every submission is timestamped and attributed to the QR code that produced it, so you can filter by code and tell a trade-show scan from a packaging scan. The inbox exports to CSV.

Which plan includes them?

The Business plan. Review Collection, Wholesale Inquiry and Loyalty Reward are Business-tier QR types; the free plan covers dynamic URL codes and a Link Hub.

Can I change the form after the code is printed?

Yes. These are dynamic codes: the printed image points at a redirect you control, so you can reword the prompt, rename the product or reward, or switch the extra fields on and off without reprinting anything.

Is the email address always required?

It depends on the flow. On a review form the email is optional, because a rating and a comment are useful on their own. On a wholesale inquiry and on a loyalty claim it is required — the first needs a reply address, the second trades the reward for a contact.

How is consent handled?

Every hosted form carries an explicit consent checkbox stating that the details are stored so you can get back in touch, and the consent flag is saved with the submission. Scans are recorded with hashed IPs rather than raw addresses, on EU infrastructure.
