Rating plus comment
The review flow asks for a score on a five or ten point scale and an open comment. The email address is optional there, because a rating and a sentence are useful on their own and asking for contact details suppresses responses.
Or your own fields
The lead capture flow renders the fields you define on the code, alongside the email and consent inputs the platform always adds. Answers are stored as a structured record, not as a blob of text.
Answered on the phone that scanned it
The form is served from the code short address, so there is nothing to install and nothing to log into. That is the difference between a response rate measured in percent and one measured in fractions of a percent.
Consent is explicit
A checkbox states plainly that the details are stored so you can get back in touch, and the answer is saved with the submission. Scans behind it are recorded from a hashed IP, never a raw address.
One code per location
Give each table, ward, branch or delivery route its own QR code and the answers arrive already segmented. Filter the Leads inbox by code to compare them without adding a question about which branch someone visited.
Change the questions without reprinting
The prompt, the rating scale and the extra fields all live on the dynamic code. Reword a question at any point and the cards, stickers and receipts already in circulation pick it up.
Bot submissions filtered
A hidden honeypot field, a per-address rate limit and a 4 KB body cap keep automated noise out of the results. A trapped submission is answered with the ordinary thank-you page.
Know the limits
This is short-form feedback: one page, no branching logic, no scoring model, no panel management. For a long questionnaire, point a dynamic code at your survey tool and keep the scan analytics on this side.
Frequently asked questions
Do I need to connect a store to use these QR codes?
No. Review, wholesale inquiry and loyalty codes serve a page hosted by QRCode Suite at the code’s own address. There is no store to connect, no plugin to install and no CRM to wire up — which is what lets you print one on a carton or a receipt for a channel you do not control.
Where do the responses go?
Into the Leads inbox in your dashboard. Every submission is timestamped and attributed to the QR code that produced it, so you can filter by code and tell a trade-show scan from a packaging scan. The inbox exports to CSV.
Which plan includes them?
The Business plan. Review Collection, Wholesale Inquiry and Loyalty Reward are Business-tier QR types; the free plan covers dynamic URL codes and a Link Hub.
Can I change the form after the code is printed?
Yes. These are dynamic codes: the printed image points at a redirect you control, so you can reword the prompt, rename the product or reward, or switch the extra fields on and off without reprinting anything.
Is the email address always required?
It depends on the flow. On a review form the email is optional, because a rating and a comment are useful on their own. On a wholesale inquiry and on a loyalty claim it is required — the first needs a reply address, the second trades the reward for a contact.
